MERC staff that are involved in, or who closely observe, an incident or near miss are required to fill out an INM form as soon as reasonably practicable.

Complaints – whether health, safety, and well-being related or otherwise – should be made by via a MERC Complaints Form. 

Staff are to fill-out a Conflict of Interest Declaration of any existing relationships with children, families, or clients; business dealings; investments; etc. that could constitute a conflict of interest, place the staff member in a position of compromise, or negatively effect MERC in any way.
